Antagonista selectivo de los receptores AT1 de angiotensina II. Su pico de acci\u00f3n ocurre a las 2-4 horas, su vida media es de 9 horas. El efecto antihipertensivo m\u00e1ximo se logra en 2 a 4 semanas. Se elimina en un 70% por v\u00eda hep\u00e1tica. Los alimentos disminuyen un 50% su biodisponibilidad.<br \/>\nPOSOLOG\u00cdA<br \/>\nLa dosis se establecer\u00e1 individualmente de acuerdo con el criterio m\u00e9dico y cuadro cl\u00ednico del paciente. Por v\u00eda oral, en una sola toma diaria, alejado de las comidas. HTA: 80-160 mg. Dosis m\u00e1xima 320 mg por d\u00eda. Insuficiencia card\u00edaca dosis de inicio 40 mg por d\u00eda, mantener con 80-160 mg cada 12-24 horas. IAM: dosis de inicio 20 mg por d\u00eda, hasta llegar a 160 mg cada 12 horas. Pediatr\u00eda: mayores de 6 a\u00f1os dosis de inicio 1,3 mg\/kg por d\u00eda. Dosis m\u00e1xima 160 mg.<br \/>\nCONTRAINDICACIONES<br \/>\nEstenosis bilateral o unilateral de la arteria renal. Coadministraci\u00f3n con aliskiren en insuficiencia renal o diabetes.<br \/>\nPRECAUCIONES<br \/>\nEmbarazo: Categor\u00eda D: hay evidencia de riesgo fetal, usar como \u00faltimo recurso. Lactancia: Potencialmente riesgoso a dosis habituales. Comenzar con dosis bajas e incrementar lentamente en hipovolemia, o estenosis de arteria renal (existe activaci\u00f3n del sistema renina angiotensina) evaluando los niveles de creatinina. En la insuficiencia renal pueden provocar hiperpotasemia. Por su forma de eliminaci\u00f3n en general no requieren ajustes significativos de la dosis excepto en casos severos de insuficiencia renal. En adulto mayor valorar funci\u00f3n renal y situaci\u00f3n de hipovolemia o uso de deplectivos.
